The 3 Components of an RVU

These RVUs offset the cost of maintaining a practice, including:
- Salaries of the personnel

Facility and non-facility settings have different practice expense RVUs since overhead varies.
Malpractice RVU (mRVU)
This accounts for the cost of malpractice insurance tied to performing the service.
The procedures with high risk have naturally higher malpractice RVUs.
RVUs Are Translated into Payment
RVUs themselves are just values—they don’t equal dollars until Medicare applies the conversion factor (CF)**.

Total RVU × Conversion Factor × Geographical Adjustment = Medicare Reimbursement
Step 1: Add the RVUs
wRVU + peRVU + mRVU = Total RVU
- Apply the Geographic Practice Cost Index (GPCI)
- Areas with higher costs of living receive higher GPCI adjustments.
- Multiply by the Conversion Factor
- The CF is updated annually by CMS.
